Documents to be presented by consumers in case of:

Loss of food book

• A member of the nucleus over 16 years of age with their identity card.
• Stamp stamp worth $ 10.00.
• Replacement for deterioration of the passbook for food products
• A member of the nucleus over 16 years of age with their identity card.
• Notebook for Food Products.
• Stamp stamp worth $ 10.00.

Consumer transfer

Documents to be presented by consumers who move from another OFFICE:

• Identity card of the appearing party.
• Identity Card, updated by the address of the house where the discharge will take place, of all the components of the nucleus.
• Model 6 of Withdrawal from the OFFICE where it was withdrawn.
• Notebook for Food Products, in the case of recognition of the nucleus due to a change of address.
• Some document that proves the legal occupation of the house, in the name of the person requesting to be head of the nucleus, issued by the Institute of Housing and Notaries.
• If the document of the legality of the dwelling is in the name of a minor under the age of 16, it will also be necessary to present the document that accredits the applicant as guardian of the minor and the Identity Card of said guardian updated by the address of the dwelling of which the minor owns.
• Stamp stamp worth $ 5.00

In cases of home sale and purchase, the nucleus of the seller will be canceled and the nucleus will be constituted to the tester.

Registration in the Consumer Registration Unit can be processed by any consumer, whether or not they are a component of the nucleus. Through this procedure, a consumer is registered in the notebook for food products.

Documents to present for consumer registrations

• Identity card of the appearing party.
• Child's Card updated by the mother's address.
• Notebook for Food Products.
• In all cases of birth the child will be registered in the notebook where the mother is registered.

Registration of people from abroad.

• If it is a child born abroad to Cuban parents or Cuban and foreigner, they will present the minor's card, to consider themselves a Cuban citizen.
• If it is a consumer of Cuban nationality and was sent abroad by an Organization, it will present an Identity Card updated by the discharge management.
• In case it has been sent by an Organism, it will present an Identity Card updated by the discharge address.
• Model 6 of Withdrawal from OFICODA where it was withdrawn before leaving the country, if it has one.
• Notebook for food products from the nucleus where it will cause discharge.

Graduates of penitentiary centers, hospitals or asylums.

• Identity card of the appearing party.
• Identity card of the person who is going to be registered, updating the address of the house where the registration will take place.
• Document from the Hospital center, Asylum.
• Notebook for Food Products where it will cause discharge.
• Model 6 Withdrawal from the Office where it was withdrawn.

Family doctors and nurses.

• Identity card of the appearing party
• Identity card of the person who is to be registered, updating the address of the home where the registration is to take place.
• Document issued by the Municipal Directorate of Public Health duly signed and coined.
• Stamp stamp for $ 5.00.
• Model 6 of Withdrawal from OFICODA where it was withdrawn.

Withdrawals may be partial or total due to:

• • Transfer to another OFFICE.
• • Death.
• • Exit abroad.
• • Income in penitentiary centers, hospitals or asylums.

Notes:

In order to carry out the OFICODA withdrawal process, the presentation of a component of the nucleus with 16 years or more is an essential requirement, an end that will be justified through the presentation of the identity card and the food products booklet.

In the cases of death, departure abroad and income in penitentiary centers, asylums and hospitals (permanently), the following must be presented:

• • Identity card of the appearing party.

Food Products Notebook

Medical diets

To process a diet you must present:

• Notebook for Food Products.
• Form 12 Sworn Declaration of Medical Diet, duly signed and coined by the Public Health Assistance Center that issues it.
• The renewal of the medical diet may be processed by the consumer, any day of the month in which the current diet expires.

Delivery of baby book

Documents to present:

• Identity card of the appearing party.
• Notebook for Food Products.
• Sworn Declaration of Medical Diet, signed and coined by the Public Health Assistance Center that issues it and must state the pregnancy time that the pregnant woman is 26 weeks or more.
